Damage Claims

If you wish to submit a claim against the City, please complete a City of Poulsbo Claim for Damages Form. The forms are also available at City Hall. The form must be signed in front of a Notary (the form can be notarized free of charge at City Hall). Once completed and notarized, the form should be submitted to the City Clerk with all bills relating to the claim attached to it.

The City will either:

  • Pay the claim
  • Forward the claim to the City Council for consideration
  • Send the claim to the City's insurance carrier for consideration
  • Reject the claim.
