Passport Services

The City of Poulsbo is an offical Passport Acceptance Facility, providing general information on passport procedures and serving as liaison between applicants and the U.S. Passport Services Agency. The actual decision to issue or deny a passport is made only by the U.S. Passport Services Agency, not the City of Poulsbo.

The City Clerk's Office provides passport services on Tuesdays by appointment only. We are located at 19050 Jensen Way NE, telephone (360) 779-3901.

All Applicants must appear in person. Notarized or unofficial photocopies of official documents (e.g. birth certificate, marriage certificate, adoption decree) are NOT acceptable. The documents submitted must be the original or certified copies from the issuing agency.

Children Under 16

  • Must present evidence of parental relationship (e.g. birth certificate, adoption decree) IN ADDITION to citizenship evidence (e.g. passport, birth certificate).
  • Are affected by the Two-Parent Consent Law.
